Ready: Initial Steps

First, let’s get to know each other. Book a virtual consultation to discover if ketamine infusions can be your portal to transformation. We will invite you to complete a quick information sheet that will give us a little bit of background on your situation and prior knowledge of ketamine. Our patient care coordinator will then contact you to schedule an initial video visit with Dr. Herman. The coordinator will be able to answer any logistical questions you have at that time. If you’re already working with a mental health provider and give us consent to consult with them, we’ll do that prior to your meeting with Dr. Herman. If you don’t have a current provider for us to partner with, we’ll invite you to meet with our board-certified psychiatrist for pre-journey guidance. After we get acquainted, answer your questions, and identify your intentions, we’ll design an individualized treatment course to bring you back to your foundation, values, and goals. Reflect: Preparation for Treatment

Your mental attitude or mindset as you embark on a ketamine treatment journey is commonly referred to as “set.” This is the foundation for your journey. By cultivating an optimal mindset prior to your inner exploration, you prime yourself to realize the treatment’s full benefits. Like all travels, this inner voyage requires a bit of advance planning and preparation. We suggest you take the following steps before we get started:

  • Clarify your intention. You’re about to travel deep into the layers of your mind and discover just how much of it you may have missed before or forgotten. To get the most out of your treatment, it helps to set off with a clear intention. Consider which issues or concerns you want to explore. Your mental health provider or our in-house psychiatrist can help you focus your treatment intention.
  • Come with an open mind. This may be a more intense experience than you have imagined. You’re here because you’re seeking something—transformation. As you start this exploration, know that powerful experiences lead to meaningful changes. Be ready to go with them as they come.
  • Come with an open heart. Don’t shy away from personally meaningful experiences on your journey. They’re your stepping stones to transformation. The emotional system of the brain is our strongest learning system, but it’s also fully implicit. That means what you experience emotionally during your treatment can have lasting effects. You can reap rewards faster by embracing the awareness of your feelings and taking time to consciously reflect on their significance during the session and after.
  • Choose your music. This journey is in part a practice of listening—to yourself, your breath, your soul, and the music that can connect you to them. Music, like the breath, has the power to change both your mental and physical states. We’ve all experienced moments when songs take us back to a place, time, or feeling. And research shows music can evoke and attune not only our moods and mind states but also our heart rate, breath rate, and blood pressure. We invite you to choose music that speaks to you.   • Begin with breathwork. You may have never given your breath much thought. Breathing is the most fundamental of physiological functions but also the most profound. Our bodies know how to do it on their own—kind of. But how you breathe is directly connected to your mood state and nervous system. When you’re stressed, anxious, fearful, or angry, your breath tends to become rapid and shallow. The upside of that is that consciously changing your breathing can radically change your state of mind. (Science has confirmed this!)
    Breathwork, or the practice of various patterns of in- and out-breaths, has the power to alter your mood, your resilience, and your state of consciousness. It can help you break through blocks and guide you to feelings of connection, expansion, clarity, and joy. We offer the option for breathwork as part of our ketamine treatment preparation or during a session to center you in the present moment, connect body and mind, ready your mindset, and shift your emotional state.

Reset: Your Ketamine Treatment Session

You’re wondering what you can expect during your ketamine infusion. Dr. Herman’s focus is to ensure that each ketamine infusion session is not only personally meaningful and therapeutically significant, but also safe and comforting. You will be immersed in specifically curated sounds, scents, and sights, with the goal of encouraging a transformative experience.

The Setting

The environment or ambience of the physical space where you set out on your ketamine journey influences your experience.
To facilitate release during a reboot session, we’ll make sure:

  •  You’re safe. You only need to focus on transformation. Throughout your entire treatment, a Harvard-trained anesthesiologist will closely monitor your vital signs with state-of-the-art equipment so you can focus on inner exploration.
  • The space is inviting. We’ll offer you Bose noise-cancelling headphones and a Mindfold eyeshade to encourage inward travel while you listen to your preferred music or guided meditation. If you would rather listen to ambient music on a speaker, that’s available too.

The Therapy

Ketamine infusion therapy is a continuously evolving science. To maximize our patients’ outcomes, we stay current with ongoing research, attend conferences, discuss our protocols with our colleagues in the ketamine therapy field, and most importantly, LISTEN TO CLIENT FEEDBACK.

Our current protocol is to administer an induction series of six infusions over three weeks. Research shows that starting therapy with six infusions results in longer lasting symptom reduction. However, we will customize the number of initial infusions based on your clinical response and any feedback you and your trusted mental health provider may offer. We firmly believe in the importance of a team approach in individualizing a client’s experience, and that includes prioritizing your own feedback on how the treatment is going for you.

After the initial series, the reduction in depressive symptoms lasts approximately five weeks, with a range of three weeks to two months noted for most people. Subsequent single maintenance (or “booster”) infusions may be necessary to sustain symptom improvement. Each patient is unique; these boosters may be indicated anytime from three weeks to three months after the initial series. We will personalize the treatment schedule to best suit you and your goals.

Ketamine-Assisted Psychotherapy

We also invite you to try ketamine-assisted psychotherapy with our in-house specialist or bring the mental health provider of your choice to our office to accompany you on your journey.

Transform: Post-Treatment Integration

Ketamine is a powerful, paradigm-shifting tool. However, it is NOT intended as a one-time treatment or cure. It is better seen as a catalyst; it functions synergistically with traditional mental health therapy. We find that scheduling treatment sessions 48–72 hours apart is beneficial to facilitate integration and introspection after your ketamine experience. You can do this post-treatment processing alone or with the support of our in-house therapist or your trusted mental health clinician. Research suggests that cognitive behavioral therapy can extend the antidepressant effects of the treatment, thereby reducing the frequency of maintenance infusions needed. To crystallize the therapy’s transformative effects, it’s vital to take intentional steps after your treatment to digest, embody, and apply the insights and discoveries from your inner exploration into your life. Consistent, ongoing follow-up with your primary doctor and/or mental health provider during and after ketamine therapy is paramount to successful treatment, and creates the best chances for long-term recovery.

We use the Osmind platform and mood-tracking app to stay in touch between appointments. You can use it to send us secure messages during business hours. We’ll ask you to track how you’re feeling between visits. This allows us to follow your response and progress after each ketamine infusion using results from brief patient surveys, as well as adjust the doses for following sessions. Such Measurement-Based Care (MBC) enhances ketamine therapy’s effectiveness and efficiency by improving patient satisfaction, outcomes, and adherence to the treatment plan. Journaling is another productive way to gather and record thoughts and lessons from the treatment experience. You can use Osmind for that, too.
